Consent Forms

All Symphony HEB participants — and, for minors, a parent or legal guardian — are asked to complete the applicable form(s) below before taking part in rehearsals, performances, or other Symphony HEB events.

Photo, Video & Media Use Release

For participants 18 and older. Covers consent for Symphony HEB to photograph, video, and audio record you at events, and to use your name, likeness, and voice in promotional and archival materials.

Minor Participant Guardian Consent & Acknowledgment

Must be completed by a parent or legal guardian for any participant under 18. Covers consent to participate, assumption of risk, photo/video/media use, and emergency medical authorization.

Completed forms can be returned to the Symphony Manager or brought to your first rehearsal.
